<p><strong>Service Coordinator</strong></p><p><strong >Program: </strong><span >Children and Youth Services</span></p><p><strong>Status: </strong>Full-time, 12-month contract</p><p><strong>Hours: </strong>36.25 per week</p><p><strong>Salary:</strong> $62,913 - $79,732 annually</p><p><strong>Date Posted:</strong> September 8, 2025</p><p><strong>Closing Date:</strong> October 10, 2025</p><p><br /></p><p><strong>Position Description</strong></p><p>Children and Youth Services at Surrey Place is seeking a Service Coordinator. Reporting to the Manager, Service Coordination, the Service Coordinator will provide service coordination to families with children and youth with developmental disabilities between the ages of 0 to 18 years. For this position Surrey Place is seeking a Service Coordinator to increase services available to Indigenous clients and their caregivers/families. The successful application will provide coordinating services both in Surrey Place and in the community. Surrey Place is client-centered, interdisciplinary and team oriented in service delivery. </p><p><strong>Main Responsibilities</strong></p><ul><li>Provide culturally sensitive services that consider the experiences of Indigenous, First Nation and Metis communities.</li><li>Assesses and collaborates with families and determines what services they require.</li><li>Make referrals to other community agencies and services as required.</li><li>Incorporates information from families and provides information to the interdisciplinary team, and/or other sources both internal and external as required in the preparation of service plans.</li><li>Support priority or urgent client cases.</li><li>Provides information to clients, caregivers, service providers and staff on the availability and appropriateness of community services and funding support.</li><li>Participates in service coordination group meetings, chairing case conferences, and community tables and meetings as required, as well as engage in community outreach to develop a collaborative approach to service delivery. </li><li>Supports and delivers Surrey Place wellness services. </li><li>Develop positive relationships with community groups, partner agencies and resources in the larger community. </li></ul><p><strong>Qualifications</strong></p><ul><li>Undergraduate degree in social or health sciences or equivalent.</li><li>3 years of related experience working with children and youth with developmental disabilities and their families, with a focus on community engagement.</li><li>Knowledge of developmental services and educational, community, and health care service systems for children with developmental disabilities.</li><li>Excellent communication and interpersonal skills.</li><li>Experience on an interdisciplinary team and with multicultural environments is an asset.</li><li>The successful candidate must provide an acceptable Vulnerable Sector Screening.</li><li>Proficient with Microsoft Office.</li><li>Preference will be given to First Nations, Inuit, or Métis candidates.
