Our pharmacy is currently seeking a licensed and registered pharmacist with the Ontario College of Pharmacists to join our team, focusing on providing patient coaching and management for medication checks, diabetes management, asthma, COPD, and other health programs.

Responsibilities:


Practice within provincial guidelines and uphold the Code of Ethics and Standards of Practice.

Provide comprehensive counseling on all prescriptions and health topics to ensure positive patient outcomes.


Perform expanded scopes of practice, including minor ailments, medication reviews, and pharmaceutical opinions.

Conduct detailed medication checks and patient coaching sessions, particularly focusing on chronic disease management (diabetes, asthma, COPD).


Collaborate effectively with allied healthcare professionals to ensure holistic patient care.

Maintain accurate records in accordance with legislative requirements.


Actively engage in continuous professional development and training programs to stay current with industry trends and best practices.

Develop and implement individualized care plans based on patient needs and health goals.


Educate patients on lifestyle modifications, medication adherence, and self-management techniques.

Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.



Qualifications:

Valid license and registration with the Ontario College of Pharmacists.


Excellent verbal and written communication skills.

Strong interpersonal skills with the ability to build rapport and trust with patients.


Proficiency in using healthcare technology for patient management.

Proven problem-solving skills with a patient-centric approach.


Ability to work collaboratively within a multidisciplinary healthcare team.

Experience in patient coaching and chronic disease management is an asset.


Strong organizational skills and attention to detail.

Ability to multitask and manage time effectively in a fast-paced environment.


Demonstrated commitment to patient safety and quality improvement initiatives.

Knowledge of current pharmacological and therapeutic principles.


Capability to provide compassionate and empathetic patient care.

Ability to educate and mentor pharmacy team members and patients.
